Issue:

With reference to the Catastrophic haemorrhage page (Page 36); it is detailed that for a catastrophic limb bleed you are to 'Apply the tourniquet on the limb as low as possible proximal to the bleeding point'. This was changed by the Military BATLS (Battlefield Advanced Trauma Life Support) training team years ago following reviews of casualties with these injuries on operations. It was found that placing a tourniquet proximal to the bleeding point which could be on the Radius/Ulna or Tib/Fib would not necessarily get the compression needed to occlude the haemorrhage as you are compressing two bones rather than the haemorrhage itself. It has now been changed to placing a tourniquet on a single bone only and if this doesn't work placing another proximal to the first
